Fitness Membership Reimbursement

Single Membership:
Continental will reimburse up to a maximum of $20 per month per employee

Single Membership Example:
Membership Fee = $15 – Continental Reimburses $15

Family Membership:
Continental will reimburse up to a maximum of $40 per month per household

Family Membership Example:
Membership Fee = $60 – Continental Reimburses $40

Online Membership:
Continental will reimburse up to a maximum of $20 per month per household

Online Membership Example:
Membership Fee = $30 – Continental Reimburses $20

Note:
Reimbursements must be submitted within 30 days of the end of the month/quarter or they may not be accepted. If you submit monthly you have 30 days after month end to submit. If you submit quarterly, you have 30 days after quarter end to submit.
